sure to put your full name
and practice postcode.

Fill in the customer data
form, enclose this with your
first case. Attach royal mail
label to package, send it to us
with royal mail (or contact us
to see your options).
3.
We will send you physical
lab dockets, adhesive post
labels, and all relevant
certificates in the post after
we have received your first
case.
4.
Log on to our website and
set up an online account.
Here you will be able to view
and check the status of your
ongoing cases, view previous
cases, and download your
invoices and statements.
